To prepare for your first visit, we ask that you complete our new patient forms. These provide helpful medical background for your doctor, ensure that we have the important insurance information we need to file your claim, and help us meet federal guidelines for HIPPA privacy protection.

Payment is expected at the time of your visit. We are contracted with many insurance companies, but please check with your insurance company to see that you are covered and which lab they will allow you to use. Please bring your insurance card or cards to your first visit so that we many copy them for your chart.

Remember to complete these forms before the date of your appointment to save time during the check-in process.
